Creating Healthier Homes
Your home can have a bigger impact on your health than you may realize. For people living with asthma, things like mold, pests, and poor indoor air quality may contribute to symptoms and make it harder to breathe.
Through New York State’s New York Health Equity Reform (NYHER) initiative and regional Social Care Networks (SCNs), eligible Medicaid members can connect with services that address health-related needs, including certain conditions in the home.
If you have New York State Medicaid, you or your family may be eligible for help at no cost to you.
Halco Home Solutions works with Social Care Networks across Upstate New York to help eligible members create safer, healthier living environments.
Struggling With Asthma? Your Home May Be Part of the Solution.
Every home, and every person, is different. If you qualify, your needs will be evaluated to determine which services are appropriate for you.
Depending on eligibility and your individual needs, services may help address conditions such as:
Mold & Moisture
Addressing moisture problems, water leaks, or conditions that contribute to mold growth.
Pests
Sealing openings and addressing home conditions that allow pests to enter or thrive.
Indoor Air Quality & Ventilation
Improvements that may help reduce indoor pollutants, allergens, and other environmental triggers.
Heating, Cooling & Humidity
Certain medically necessary improvements or equipment may be available to help create a healthier indoor environment.
Asthma Triggers
For eligible individuals with asthma, services may include a home environmental assessment, education, supportive products like a HEPA vacuum, asthma-friendly cleaning products, and pillow and mattress covers, and home improvements designed to reduce asthma triggers.
Not every individual will qualify for every service. Your Social Care Network will determine eligibility and which services are appropriate for your needs.

Social Care Networks help coordinate these services so members can get connected with the support they need.
